ACSM-CEP certification, exercise as clinical care
The ACSM Certified Clinical Exercise Physiologist (ACSM-CEP) credential is for exercise physiologists who test and train patients with heart, lung, metabolic, cancer and other chronic disease. You apply before you can book: every application is audited for the degree and 600 h or 1,200 h of clinical work. The exam itself is 115 questions, 100 of them scored, in 3 h 30 min.
Clinical exercise physiologists who test and prescribe exercise for people with cardiovascular, pulmonary, metabolic, orthopedic, neuromuscular, neoplastic, immunologic or hematologic disease, from children to older adults

Passing score
Scaled score of 550Forms are equated, so the number of correct answers behind a 550 varies.
55% of first-time candidates passed in July–December 2025, down from 65% in 2024Repeat candidates 49%. 2024: 65% first-time, 47% repeat; 2023: 60% and 42%.

Continuing education
60 continuing education credits (CECs) every 3 years; CPR/AED and first-aid courses count for up to 5 CECs a year
The credential is written for cardiac and pulmonary rehab, hospital exercise testing and medically supervised programs: patients arrive with a diagnosis, a medication list and often a referral. Hours spent with athletes, apparently healthy clients or personal-training clients do not count toward eligibility, which says plainly who it is for.
Training healthy adults or controlled disease? The ACSM-EP is the rung below. It asks for exercise science bachelor’s + CPR/AED, no clinical log, and has 140 questions. The two side by side: exercise physiologist certification.
Coaching athletes? Performance training is CSCS territory, and the physiology both exams lean on lives on one page here: CSCS exercise science.
Already hold an RCEP?The RCEP, the older CEP and the ETT were consolidated into today’s ACSM-CEP; earlier holders keep a legacy designation that cannot be renewed.
Six domains in the July 2025 outline
Exercise training and leadership is the largest domain at 24%; legal and professional responsibilities is the smallest at 5%, about 5 scored items. Each domain gets a page and a question set.
The July 2025 outline puts more weight on clinical exercise testing, risk management and multidisciplinary care, and the first-time pass rate fell from 65% (2024) to 55%. Repeat candidates passed at 49% over the same half-year. A guide written before July 2025, or built on GETP 11, describes the outline that was retired: what changed.

Question 1 of 4 · Education and Behavior Change · 13%
A CEP provides discharge instructions on recognizing angina symptoms. Which request best verifies patient understanding using the teach-back method?
A signature shows the handout was received, not that the patient can recognize angina.
Many patients say no even when confused; this closed check does not make them explain anything.
A yes/no promise invites agreement and reveals nothing about what the patient actually knows.
Teach-back asks the patient to say the plan back in their own words, so the CEP hears whether the warning signs were understood.
Self-rated confidence is not comprehension; the patient never shows what they would do.
Question 2 of 4 · Exercise Testing · 18%
A patient reports angina pattern changes over the last 48 hours, now occurring at rest. Which action is required regarding the scheduled exercise test?
Unstable angina is on the absolute list, so it is not a case for weighing benefit against risk.
A pharmacologic stress test also provokes ischemia and is likewise deferred until the unstable state is treated.
Angina that has changed pattern and now occurs at rest is unstable angina, which rules out exercise testing until the patient is stabilized.
Relieving the pain does not stabilize the plaque; testing someone with rest angina remains unsafe.
Physician presence does not make testing safe in unstable angina; the contraindication stands regardless of who supervises.
Question 3 of 4 · Exercise Prescription · 22%
Which exercise modality is most appropriate for a patient with autonomic dysfunction who experiences severe orthostatic intolerance?
Upright walking lets blood pool in the legs, which is what provokes orthostatic symptoms.
The reclined position keeps venous return up and the head near heart level, so orthostatic symptoms are least likely.
Seated upright still places the torso vertical, so venous pooling is greater than in the recumbent position.
Standing exercise with a moving base adds pooling and a fall hazard if the patient becomes lightheaded.
Upright, weight-bearing climbing is among the worst choices for someone who becomes symptomatic on standing.
Question 4 of 4 · Patient Assessment · 18%
Which of the following medications would MOST significantly affect resting heart rate assessment?
Acid-suppressing drugs do not meaningfully change heart rate.
Beta-blockade lowers resting heart rate and blunts the exercise rise, so resting and target rates must be read with the drug in mind.
Common antihistamines have little effect on resting heart rate at usual doses.
Statins act on cholesterol synthesis and do not alter resting heart rate.

A Certified Clinical Exercise Physiologist: an allied health professional who tests and prescribes exercise for people with chronic disease. Getting there takes a degree, 600 h or 1,200 h of clinical experience, a passed audit and a passed exam.
Is the ACSM-CEP the same as the RCEP?
It replaced it. The RCEP, the earlier CEP and the ETT were folded into one credential, today’s CEP; RCEP holders keep a legacy title that cannot be renewed, and new candidates sit the CEP.
How hard is the ACSM-CEP exam?
55% of first-time candidates passed in July–December 2025, against 65% in 2024. Every published year: pass rate.
Can I take it online?
Yes, through Pearson OnVUE. The audit still comes first: no one books a seat before the application is approved.
